The criteria to become a member of CAAP are children and young people aged 0-25 years who have (or had) a statement/EHCP to attend a mainstream school or go to a special school, ARB or PRU.
If you would like to register your child/young person aged 0-25 with CAAP then please download the form and return it to us either by email or by post to the address found in the contacts page making sure you answer all questions and sign both sides of the form.
(Please note that if your child/young person does not have (or had) a statement/EHCP, attend a special school, ARB or PRU then you will need to have the referral form signed by a professional involved in their care).
